Students enrolled in Clinical Education are required to update the following items annually:
- Please review the presentations and complete the corresponding quizzes.

and FERPA-

completing the HIPAA/FERPA Training
requirement.

Sexual Harassment Training
- If you have already completed the training, you MUST update your knowledge.
- All students should provide evidence
of completion (print and scan, or print
screen your completion certificate).
- Please be reminded, you will need to complete a
Criminal Background Check and Fingerprinting when you arrive to campus.

- Please scan and email a copy of your new CPR
card to Dr. Eberman (leberman@indstate.edu)
(If your card has expired or will expire during the Fall
semester.)
- If you have purchased professional liability
insurance, please email a copy of your
certificate to Dr. Eberman (leberman@indstate.edu).
- Professional liability insurance is NOT required, unless you plan to participate in extra-curricular activities (i.e Preseason camp [without enrollment in class], Research activities, Ironman competitions, etc.) or exceed the 20 hours limit on clinical education experiences.
