Prescription Coverage
ANTHEM PRESCRIPTIONS

Effective January 1, 2005, ISU will utilize the services of the State’s
program administrator, Anthem. There is no change in the
prescription drug plan design with the Anthem contract. You should
be able to continue to use your current pharmacy and purchase the same
drugs that were covered in 2004. Anthem also has a mail order
program . There may be slight differences in payment due to the
State’s contract and pharmacy discounts. However, it is the hope
of the State of Indiana and Indiana State University that the statewide
pool will provide deeper discounts and savings for everyone.

Mail order pharmacy:
You can call 1-800-962-8192 for questions regarding the mail order plan. Anthem RX Mail Order Form.
Important: If you need to purchase a prescription, you will need your identification number that is on your Anthem prescription drug card and your date of birth. Be sure to tell your pharmacist that your coverage has changed to Anthem. Your new group number is 00084240.
