The open enrollment period for the health, dental and prescription drug card coverage is during the month of November 2009. Applications received during November will have health, prescription drug card and dental coverage effective January 1, 2010. Open enrollment is offered each November for eligible employees to enroll, or to enroll an eligible dependent.
If you wish to enroll in the coverage or add an eligible dependent, please complete the “Open Enrollment Application” and a Salary Conversion Form. Return the application and Salary Conversion Form to the Staff Benefits Office. All applications must be received on or before November 30, 2009. Any applications received later than November 30, 2009 will not be approved for this open enrollment. If this is a new enrollment, you will also need to complete a Salary Conversion Form. When adding an eligible dependent, documentation must also be submitted.
The plan utilizes a network of participating providers and covers one eye exam per 12 months, one set of lenses per 12 months, one set of frames per 24 months, and contacts once every 12 months (in lieu of lenses & frames). Additional items chosen above basic glasses or contacts must be paid by the employee. Special frames, tinting and other enhancements will also cost extra. You may use a provider that is not in the UnitedHealthcare Vision network and be reimbursed based upon a reimbursement schedule for such purchases. ENROLLING IN COVERAGE:
To enroll in the UnitedHealthcare Vision coverage, please complete the
enrollment and
salary conversion
forms and return the forms to the Staff Benefits Office on or
before November 30, 2009. Your University ID number must be used on your
enrollment form and should be used as your identification number to access your
coverage with providers. Enrollment
forms are also available on the Staff Benefits website.

Section 125 of the Internal Revenue Code allows employees the opportunity to pay
for health and vision benefits on a pre-tax basis.
Annually, employees can elect to make a change for the upcoming calendar
year. Now is the time for employees
to make change decisions for 2010.
When enrolled in the health or vision plan, the amounts deducted from your
paychecks for health or vision coverage, are exempt from Federal, State and
Local income taxes and Social Security/Medicare (FICA) taxes.
You can save taxes and increase your take-home pay by using this option.
Your pay that is subject to taxation is reduced by the exact dollar
amount of the premiums you pay by payroll deduction.
Participation in the Plan is voluntary and does not affect any of your other
benefits. If you earn less than the maximum amount of salary subject to FICA,
$106,800 in 2010, participation in this salary conversion program may reduce the
amount of your future monthly benefit from Social Security since you and the
University will be paying less FICA taxes.
You will also not be able to include the cost of your medical
coverage as a deduction on your Federal income taxes if enrolled in the salary
conversion program.
