21st Century Scholars

TO APPLY FOR THE PROGRAM

• Obtain an application from the student’s school or the Twenty-first Century Scholars Wabash Valley Regional Support Program office.

• Fill out the application completely in black ink, printing in capital letters.

• Return the completed application by the June 30 deadline to Twenty-first Century Scholars Program, 150 W. Market St., Suite 500, Indianapolis, IN  46204.

• OR complete an application online.
